As a writer I often seem to read about a story idea that is so obvious, yet so brilliant, that it seems like even a monkey poking at a typewriter could turn it into an award winner. But what amazes me even more is when the narrative itself is written in a clever way. So I present the five that have most wowed me. Some of the techniques in this list now seem hackneyed, but when they were first used they shocked readers and confounded critics. Others are far less widely-used these days. While one can probably never be used again. WARNING: This piece unavoidably contains spoilers.

“Boutique” or “retainer” medical practices have been steadily growing since 2005. In this practice model, patients pay an annual retainer fee outside of insurance to gain greater access to their physician. (1)
While it is growing in popularity, some physicians, ethicists, and policy makers are concerned about the trend. (2)
“Concierge care…is like a new country club for the rich,” Representative Pete Stark, Democrat of California, said at an economic committee hearing to Congress in April 2004. “The wealthy will pay for exclusive access to quality care, and everyone else will continue to have inferior access to primary care physicians, specialists, and basic medical advice.” (3)
Proponents of concierge medicine, on the other hand, say that it enables doctors to provide the best possible care and remain in a clinical setting. Dr. Bernard Kaminetsky, an internal medicine physician in Florida, told the New York Times he would be working for a pharmaceutical company if he hadn’t been able to move to a concierge model. “I’m really helping a lot of people. I feel good about what I do,” he stated. (2)
To learn more about this growing trend, the Student Doctor Network spoke with Arney Benson of SignatureMD located in Santa Monica, California. SignatureMD helps primary care physicians transition their practice to a retainer medicine model. He is a graduate of the Massachusetts College of Pharmacy Allied Health, and has over 25 years of healthcare consulting experience. He currently serves as President of AB Consulting and Senior Vice-President for Physician Development for SignatureMD.
How do you define “concierge” or “retainer medicine”?
Retainer medicine, sometimes referred to as “concierge” or “boutique” medicine, is a different type of care delivery experience in which physicians limit the size of their patient panel in order to provide more proactive health care services and greater convenience and access to their patients. Patients pay a defined fee to experience this type of care, the specifics of which vary among physician practices.
How do retainer practices fit into the current health care structure (i.e., Medicare, insurance companies, etc.)?
A retainer practice focuses on patients in a proactive continuum of care. You get to know your patients well and help them to coordinate their healthcare. Instead of building your practice up to a panel with thousands of patients, you will have a panel between 300-500 patients. While you can still accept insurance, you will also assess a yearly membership fee from your patients.
This retainer model typically requires fewer supporting personnel because of the lower patient load. Therefore, you will have fewer patients and fewer staff to manage.
The retainer practice also offers a different service level that might include cell phone and/or e-mail access, same day appointments, longer physicals and routine appointments, coordination with fitness and nutrition providers and 24/7 access.
However, any practice continuing to participate in insurance plans must take into consideration the view of retainer fees by those insurance providers. When the legality of retainer medicine comes into question, it’s typically because an insurance provider has a provision that does not allow the patient to be billed a fee for such management. It is wise to work with a team of legal advisors, or a company like SignatureMD, to mitigate your risk.

Ive heard many times in many places that you shouldnt live and die simply by the scale and that you should instead plot your daily weight on an average curve that smoothes out the daily fluctuations in weight. I never had the patience to do this until recently but now that I have I fully understand the benefits.
Its also important to limit your variables by taking your weight at around the same time every day, with the same scale, under similar conditions. I do this every morning right after I get up. I was seeing my family doctor because of a car accident on August 29, 1996. He hadnt seen me for a few months and could see quite a difference immediately. All the tests he ran were much improved. Ive been on Vyvanse for almost 3 months now, 50 mg a day. Its my first time on ADHD medicine, but Ive been on Wellbutrin XL 300mg for 18 months. When I began taking the Wellbutrin I weighed about 155 lbs, in Dec of 07. Since then I got down to about 137-140 consistently, but since I started taking Vyvanse my appetite is completely dead. I cant force myself to eat, because if I do then I get so sick that I have to go to the bathroom and try to throw up. I am NOT bulimic or anorexic. Its just miserable being at work every day and feeling so nauseated, so sometimes I just sit in the bathroom and hope that I can throw up - I dont make myself by gagging or anything.
For the first time in my life I feel 110% confident in my body and am loving the way I look, but I know that if I continue not eating Im not going to be healthy. When I started Vyvanse I was average 137-140 lbs.
Today I weighed in at 122.8 lbs. Those 14 lbs were all lost within the last month, and it seems to be coming off even faster now. Ive lost 7 pounds since last week. Im 5 and 23 years old, and now my family (they dont know Im on anything) is telling me that I look skinny, but I love how I look now.
My point is - sorry I took so long to get there - that I want to know what I can consume each day to keep my nutrition from crashing, but without eating whole meals? I take vitamins, but is there something that I can put in my stomach so that at least I know Im getting some sort of healthy substance? Like any specific sources of protein or something? Even when I did have an appetite I rarely ate meat, so lack of that is nothing new. I just need to find something to put me at ease about my body and make me feel like Im not deteriorating. When I say something I can eat without eating whole meals, I really do mean that I cant eat meals. An example of what I eat per day is a latte in the morning, and sometimes - if I act as soon as I feel the hunger - something such as a bowl of cereal for dinner. That is how my health is now, and I know the obvious answer would be eat multiple small meals a day etc etc, but I cant force myself to do that. Thats why Im asking if theres something small but has lots of nutrients?
Please help with any suggestions

If you want nutrient dense, I would suggest hard boiled (or raw or softboiled whatever you prefer) egg yolks. You can survive without vegetables, fruits, grains and stuff, but you got to have protein and fat. Eggs really are a perfect food and just eating the yolks gives you a better fat ratio without adding fat. If you have lil appetite, Id rather see you eat 2-3 egg yolks (or whatever you can get down) than one whole egg (the white of an egg is not perfect but its dense) Try to eat as many yolks as you possibly can before taking a multi vitamin.
You will lose more body fat eating protein and fat (dont eat protein alone) than not eating AT ALL. To lose weight fast eat all you want but nothing but meat, eggs, healthy oils, mayo, butter and half an avocado a day (youll need added potassium). Keep your fat percentage high. Adding in green vegetables some cheese will continue weight loss but at a slower pace.
If you starve yourself, your body will freak out and will want to store every ounce in case it doesnt get nourished again. You need to give it adequate nourishment so it doesnt slow your metabolism down to adjust for lower calories.
Simple carbohydrates (sugar, flour, bread, cereal, pasta, potatoes, rice) triggers insulin which can store the calories you eat into fat. The more protein you eat the more the fat burning hormone glucagon is released. The more carbohydrate you eat the more the fat storage hormone insulin is released.
High insulin levels unbalance other hormones. Controlling your insulin level will balance out other hormones allow human growth hormone (HGH) to be produced naturally so you will gain lean muscle even without exercise. Any exercise will greatly increase your muscle mass with high HGH levels.
Reducing body fat and increasing muscle mass will have you in awesome shape in no time. Good luck.
